Arizona Transplant Associates, PC is contracted with the following insurance companies. We will submit claims on your behalf to the insurance carriers. Patients that have a medical insurance plan must bring their insurance card(s) with them to their appointment.
Most AHCCCS plans require a referral form from the primary care physician for the scheduled appointment.

It is recommended that patients verify their in-network and out-of-network insurance benefits prior to the scheduled appointment by contacting the customer service number on their card. Patients should be prepared to pay their co-payments, deductible, and/or co-insurance at the time of service. For your convenience, the office accepts check, cash, MasterCard, and VISA.
